RESUMEN La obesidad y el sobrepeso se definen como una acumulación anormal o excesiva de grasa que puede ser perjudicial para la salud. Es una enfermedad crónica de origen multifactorial, que como consecuencia ocasiona problemas higiénicos, discapacidad funcional y alteración de la calidad de vida. La dermolipectomía abdominal es una técnica quirúrgica que constituye la única opción para el tratamiento de pacientes con panículo adiposo prominente; de esa forma se logra disminuir las complicaciones y se reintegra socialmente al paciente que, a causa del enorme faldón abdominal, estuvo limitado por muchos años. Presentamos el caso de una paciente de 53 años con lipodistrofia abdominal grave, que llegaba a ambos tobillos con afectación cutánea, y discapacidad para realizar actividades diarias, resuelta con dermolipectomía abdominal y resección de 29 kg de tejido dermograso. Se realizará, además, una revisión bibliográfica del tema.


ABSTRACT Overweight and obesity are defined as abnormal or excessive fat accumulation that presents a risk to health. It is a chronic disease caused by multiple factors, which results in hygienic issues, functional disability and impaired quality of life. Abdominal dermolipectomy is the only surgical option for the treatment of patients with prominent panniculus morbidus, thus reducing complications and providing social reintegration for the patient who has been limited for many years due to the enormous abdominal panniculus. We report the case of a 53-year-old female patient with severe abdominal lipodystrophy with coverage of both ankles, skin involvement, and disability to perform daily activities, that was managed with abdominal dermolipectomy and resection of 29 kg of dermo-adipose tissue. Bibliographic research is also presented.

BACKGROUND: Excess fat and skin in the upper arms have become troublesome with aging and especially after the advancement in methods of weight reduction. Arm contouring procedures can be divided into three groups: those dealing with skin redundancy, those dealing with the lipodystrophy, and a combination of both. This study tries to find an answer to the debate about the safety of simultaneous circumferential liposuction and brachioplasty. METHODS: Sixty-two patients (49 women and 13 men) were operated on by simultaneous circumferential suction-assisted lipectomy followed by brachioplasty. Preoperative and postoperative arm circumferences and outcomes (including complications and patient satisfaction) were evaluated starting at least 6 months after the procedure. RESULTS: Only two patients (3.2 percent) developed small areas of wound dehiscence that healed after repeated dressing and an extended period of compression garment use. One patient (1.6 percent) complained of hypertrophic scarring, which was managed by local compression and silicone sheets. The average reduction in mid arm circumference was 9 cm (range, 5 to 14 cm). Approximately 95.2 percent of the patients in the study are highly satisfied, and 4.8 percent reported a mild degree of satisfaction. CONCLUSIONS: Simultaneous circumferential arm liposuction followed by brachioplasty addresses both the lipodystrophy and arm ptosis in a single hospital admission. This combination does not increase the complication rate. The results are highly satisfactory to the patients. According to the results of this study, circumferential arm lipobrachioplasty is considered to be a safe, efficient, reliable, and feasible procedure. Introdução: Atualmente com o aumento das gastroplastias redutoras (cirurgia bariátrica) e grande perda ponderal, há também um aumento na procura destes pacientes pela cirurgia plástica. Uma das características destes pacientes é o excesso de tecido dermogorduroso que causa deformidades, principalmente nos membros inferiores, acometendo a região trocantérica. Objetivo: Relatar um caso de dermolipectomia trocantérica em paciente pós-cirurgia bariátrica, onde a lipoaspiração por si só não resolveria a correção da deformidade. Relato de caso: IPMS, sexo feminino, 55 anos, histórico de gastroplastia redutora (cirurgia bariátrica) com grande perda ponderal. Relata desconforto importante com a lipodistrofia e extensa flacidez em região trocantérica bilateral. Após avaliação pela equipe, foi optado por realizar dermolipectomia trocantérica bilateral, em janeiro de 2019, no Serviço de Cirurgia Plástica Osvaldo Saldanha. Discussão: Nos casos de lipodistrofia trocantérica com deformidades graves, a cicatriz da dermolipectomia em relação à deformidade é favorável quanto a escolha desta técnica, pois a lipoaspiração poderá agravar ainda mais a deformidade, sendo mandatório utilizar a técnica de dermolipectomia trocantérica para corrigila. Quanto à lipodistrofia com deformidade moderada há dúvida entre a relação do benefício e a deformidade resultante, sendo aplicada a técnica de acordo com a necessidade do paciente e, por fim, nos casos de lipodistrofia com deformidade leve, opta-se pela lipoaspiração devido à correção ser realizada sem grandes cicatrizes aparentes. Conclusão: Portanto, a lipoaspiração tem benefício nos casos de adiposidade localizada, limitando as indicações da técnica de dermolipectomia, em especial na região trocantérica, sem invalidá-la para casos selecionados, como o descrito neste relato de caso.


Introduction: Currently, with the increase in reducing gastroplasty (bariatric surgery) and the great weight loss, there is also an increase in the demand of these patients for plastic surgery. One of the characteristics of these patients is the excess of dermal adipose tissue that causes deformities, especially in the lower limbs, affecting the trochanteric region. Objective: To report a case of trochanteric dermolipectomy in a patient after bariatric surgery, where liposuction alone would not solve the correction of the deformity. Case report: IPMS, female, 55 years old, history of reducing gastroplasty (bariatric surgery) with great weight loss. She reports significant discomfort with lipodystrophy and extensive flacidity in the bilateral trochanteric region. After the team's evaluation, it was decided to perform bilateral trochanteric dermolipectomy, in January 2019, at the Plastic Surgery Service Osvaldo Saldanha Discussion: In cases of trochanteric lipodystrophy with severe deformities, the scar of dermolipectomy in relation to the deformity is favorable in terms of the choice of this technique, since liposuction may further aggravate the deformity, and it is mandatory to use the trochanteric dermolipectomy technique to correct it. Conclusion: Therefore, liposuction is beneficial in cases of localized adiposity, limiting the indications for the dermolipectomy technique, especially in the trochanteric region, without invalidating it for selected cases, as described in this case report.

RESUMO

We present a 48-year-old female patient who presented with features consistent with acquired partial lipodystrophy (APL) also known as 'Barraquer-Simons syndrome'. It is a rare disease characterised by a gradual and progressive onset of lipoatrophy limited to the face, neck, upper limbs, thorax and abdomen and sparing the lower extremities. The resultant physical appearance can have significant psychosocial sequelae, further compounded by misdiagnosis and delay in recognition and management. Treatment is aimed at surgical correction of soft tissue destruction. Autologous fat transfer is an established plastic and reconstructive procedure that is safe and minimally invasive and can be used to reconstruct a variety of soft tissue defects and has shown to be an effective treatment modality in patients with APL.

Introdução: A lipoaspiração tem sido submetida à evolução constante desde a sua consolidação e emprego sistemático. O auxílio de tecnologia ultrassônica de terceira geração, VASER® (Vibration Amplification of Sound Energy at Resonance), se destina a facilitar a execução da lipoaspiração e trazer maior segurança e resultados satisfatórios, especialmente na busca por maior definição e lipoaspiração superficial. Métodos: No período de 2015 a 2017, 76 pacientes foram submetidas à lipoaspiração para melhora de contorno corporal no Centro Hospitalar Santa Mônica em Erechim. Foram avaliados os resultados obtidos, as possíveis complicações e a segurança do emprego do VASER®. Resultados: A utilização rotineira do VASER® gera aperfeiçoamento de resultados em contorno corporal. A emulsificação gerada pelo dispositivo associada à lipoaspiração em diversos níveis permite uma maior definição e evidenciação dos marcos anatômicos. Conclusão: Lipoaspiração associada ao VASER® permite ao cirurgião plástico o refinamento de seus resultados com a preservação da segurança do paciente.


Introduction: Liposuction has been improved continuously since it was first introduced. The third-generation ultrasound technology VASERTM (Vibration Amplification of Sound Energy at Resonance) facilitates liposuction, providing improved safety and satisfactory results, especially in the search for greater definition and superficial liposuction. Methods: From 2015 to 2017, 76 patients underwent liposuction to improve their body contour at the Santa Monica Hospital Center in Erechim, Rio Grande do Sul, Brazil. The results, complications, and the safety of VASERTM were evaluated. Results: The routine use of VASERTM improves body contour. The emulsification generated by the device, along with liposuction, resulted in greater definition and revelation of the anatomical landmarks. Conclusion: Liposuction associated with VASERTM allows plastic surgeons to refine the results better while ensuring patient safety is maintained.

BACKGROUND: Facial lipoatrophy is a rare condition described by the disappearance of facial subcutaneous fat. The etiology of lipoatrophy can be congenital, or acquired including traumatic, iatrogenic or idiopathic. Idiopathic facial lipoatrophy has only been previously identified in three case reports, among which, the patient demographics vary considerably. Two of these case reports have identified a role for autologous fat transfer as a means of treatment. This case differs from those in the literature in patient demographics and severity of the facial lipoatrophy. The aim of the current report is to present a rare case of idiopathic facial lipoatrophy, and to assess the use of autologous fat transfer as a treatment modality. CASE PRESENTATION: We present a case of a 40-year old woman from Nova Scotia, Canada who presented with asymptomatic idiopathic facial lipoatrophy. The patient was otherwise healthy, taking no medications with no trauma or surgery in the region affected. Investigations, including a full autoimmune workup, were unremarkable. The facial lipoatrophy was considerably disfiguring and was assessed as a Grade 4 on the facial lipoatrophy scale. The patient was treated over the course of 2 years with two autologous fat transfers. CONCLUSIONS: Achieving resolution of idiopathic lipoatrophy is important to patients because it can manifest in a disfiguring form and have negative effects on quality of life. The current study reports a treatment of idiopathic lipoatrophy that achieved results acceptable to the patient.

RESUMO

An increasing amount of evidence supports the benefits of fat grafting for breast augmentation, correction following breast-conserving surgery, breast reconstructions as well as correction of tuberous, hypoplastic and asymmetrical breasts. The aim of fat grafting is to create a breast with an aesthetic, natural appearance. In this review, we describe the most common indications for fat grafting of the breast and give an overview of the techniques in use as well as their associated risks and future perspectives.

BACKGROUND: The authors present the technique called "abdominoplasty with concurrent circumferential liposuction," where the whole trunk is addressed as a single aesthetic unit. METHODS: Between January of 2008 and June of 2016, abdominoplasty with circumferential liposuction was performed in 1000 patients (984 women and 16 men) by the senior author. Patients had a mean body mass index of 27.5 kg/m (range, 18.1 to 34.5 5 kg/m). Liposuction was performed circumferentially and the abdominal flap was detached up to the costal margins. Simultaneous fat grafting was also performed if required. Outcomes and complications were documented. RESULTS: Traditional abdominoplasty incisions were used in 359 patients, whereas extended incisions were used in 641 patients. The mean duration of follow-up was 27 months (range, 12 days to 61 months), and the mean duration of surgery was 154 minutes (range, 109 to 260 minutes). The mean amount of simultaneously aspired supernatant lipoaspirate was 1940 cc (range, 1700 to 4580 cc). Simultaneous gluteal fat grafting was performed in 34 percent. None of the patients had any skin necrosis. The most common complication was seroma (19 percent). CONCLUSIONS: In abdominoplasty, the entire central body should be considered as a single unit. Circumferential widespread liposuction with standard undermining of the abdominal flap up to the costal margins is safe and yields superior aesthetic outcomes. La Lipoplastia asistida por ultrasonido (UAL) es una técnica empleada desde ya muchos años. Con el desarrollo de nuevas tecnologías, se replantea constantemente la terapéutica más eficaz con los nuevos recursos disponibles, por lo que se elaboran aparatos capaces de optimizar resultados llevando a un tratamiento más innovador y eficaz. Surgen nuevos conocimientos sobre las propiedades físico-mecánicas de los aparatos con ultrasonido, lo cual abre una ventana de oportunidades para su uso médico, particularmente para el uso en la liposucción en el tratamiento de los pacientes con lipodistrofia corporal, quienes podrían ser beneficiados por las ventajas de estas tecnologías en comparación con las que actualmente están en uso. Las características de los aparatos de ultrasonido en el campo de la cirugía plástica y su evolución con las décadas ha permitido obtener mayor eficacia en su acción y ademas ha disminuido las complicaciones. Se realiza una recopilación de datos sobre la Liposucción Asistida por Ultrasonido en el tratamiento de la lipodistrofia. Se exponen las cualidades del aparato ultrasónico en la lipodistrofia, su historia evolutiva, sus logros y mejores resultados, anteriormente no alcanzados con las técnicas hasta ahora empleadas


Ultrasound-assisted Lipoplasty (UAL) is a technique that has been used for many years. With the development of new technologies, the most effective therapy is constantly being rethought with the new resources available, so that devices are developed capable of optimizing results leading to a more innovative and effective treatment. New knowledge about the physico-mechanical properties of ultra sound devices emerges, which opens a window of opportunities for medical use, particularly for the use in liposuction the treatment of patients with body lipodystrophy, who could benefit from the advantages of these technologies compared to those currently in use. Due to the characteristics of ultrasound devices in the field of plastic surgery and its evolution over the decades, it has allowed greater effectiveness in its action, in addition to reducing complications. A collection of data on Ultrasound-Assisted Liposuction is performed in the treatment of lipodystrophy. The qualities of the ultrasonic apparatus in lipodystrophy, its evolutionary history, its achievements and better results, previously unreached with the techniques used until now, are exposed

Antecedentes y Objetivo. La liposucción asistida por ultrasonido quirúrgico (USAL por sus siglas en inglés) se utiliza ampliamente en el tratamiento de las lipodistrofias corporales. En la ginecomastia, está demostrado que la combinación de ultrasonido y liposucción presenta mayor efectividad en el resultado final. Queremos dar a conocer la experiencia de nuestro Servicio en el abordaje de esta patología implementando la USAL como tratamiento único y como medida terapéutica coadyuvante de la cirugía abierta. Material y Método. Estudio descriptivo, de cohorte única, durante el segundo semestre del 2017 con registros de todos los pacientes tratados por el Servicio de Cirugía Plástica del Sanatorio Los Arroyos y Hospital Privado de Rosario (Rosario, Argentina) entre 2011-2016. En total, 43 pacientes con edades comprendidas entre 16 y 70 años evaluados retrospectivamente y estratificados según el método de corrección quirúrgica en 2 grupos. Grupo 1 (n=26) tratamiento exclusivo con USAL y grupo 2 (n=17) USAL combinada con resección abierta de tejido glandular mamario. Analizamos el tiempo de recuperación postquirúrgica, las complicaciones y la conformidad subjetiva de los pacientes. Resultados. Ninguno de los casos presentó complicaciones que requirieran intervención adicional. Como complicación menor hubo 4 casos de equimosis torácica importante (2 en cada grupo). Todos presentaron adecuada retracción cutánea y conformidad con el resultado a los 30 días. Comparativamente, encontramos amplia diferencia en el tiempo de reintegro a las actividades cotidianas tras la intervención: media de 7 días para el grupo 1 frente a 11 días para el grupo 2. Conclusiones. La USAL es una importante herramienta terapéutica en ginecomastia, de especial relevancia en casos de predominio graso dado que por si sola es un tratamiento efectivo en estos casos. En los casos de ginecomastia mixta que requieren resección quirúrgica, la USAL es una importante medida coadyuvante para un resultado postquirúrgico óptimo con adecuada retracción cutánea y mínimas complicaciones


Background and Objective. Surgical Ultrasound assisted liposuction (USAL) is widely used in the management of body lipodystrophies. In gynecomastia, it has been reported that thus combination of ultrasound and liposuction has proven an improved effectiveness and better outcomes. We present our experience in the management of this pathology using USAL exclusively and as adjuvant measure of conventional open surgery. Methods.A descriptive and single cohort study was performed during the second semester of 2017 with patients’ records from the Plastic Surgery Department of Sanatorio Los Arroyos and Hospital Privado de Rosario (Rosario, Argentina), treated between 2011-2016. A total of 43 patients diagnosed with gynecomastia, with ages between 16 and 70 years, retrospectively assessed and stratified into 2 groups according the therapeutical approach. Group 1, treated with USAL exclusively (n=26) and group 2, treated with combined USAL and open mammary glandular tissue excision surgery (n=17). Postoperative downtime, complications and patient’s satisfaction were assessed. Results. None had any major complications requiring additional measures. The only reported minor complication was significant thoracic bruising in 4 patients (2 in each group). All patients showed adequate skin retraction and 30 days post-operative results satisfaction. We found wide differences when compared the reported postoperative downtime, where group 1 reported a mean of 7 downtime days versus 11 downtime days for group 2. Conclusions. USAL is an important therapeutical measure when treating patients diagnosed with gynecomastia. It’s especially useful when treating gynecomastia with fatty dominance, being effective by itself. Additionally, when treating patients with significant glandular tissue which needs surgical excision (mixed gynecomastia), USAL seems to be an useful adjuvant measure to achieve an outstanding outcome with optimal skin retraction and minimal complication rates

INTRODUÇÃO: A obesidade é um problema importante na atualidade. Mudanças de hábitos dietéticos, exercícios físicos e a cirurgia bariátrica promovem melhor qualidade de vida. A redução ponderal excessiva ocasiona deformidades em diversos segmentos corporais que são corrigidas em múltiplos tempos operatórios. Apresentamos uma sistematização das maneiras de tratamento destes pacientes, que nos permitem atos operatórios envolvendo as várias regiões do corpo em uma mesma etapa. MÉTODOS: Estudo observacional longitudinal retrospectivo. A técnica cirúrgica consistiu em incisão do tipo asa de gaivota em dorso, com ressecção do excedente dermogorduroso associada à abdominoplastia, promovendo melhorias no púbis, dorso, nádegas e face lateral das coxas apenas por esta única incisão. A associação de excisões satélites como o em lifting em de coxas ocorreu em determinadas situações, bem como o tratamento do segmento superior do tronco em um segundo momento. RESULTADOS: A faixa etária dos pacientes variou de 24 a 52 anos, com média de 34 anos. O tempo médio do ato operatório nas excisões circunferenciais foi de 4 horas e 8 minutos. Não foram observadas complicações cirúrgicas como hematoma, necrose do retalho e/ou infecção. Três casos de pequenas deiscências e seroma, além de um caso de tromboembolismo pulmonar, foram observados. No seguimento ambulatorial, 85,7% dos pacientes mostraram-se muito satisfeitos quanto ao tamanho e posição das cicatrizes. A análise técnica apresentou um grau de satisfação de 92,8%. CONCLUSÃO: Os resultados satisfatórios elucidam a segurança e praticabilidade da técnica, sendo as cicatrizes aceitáveis devido à melhora do contorno corporal.


INTRODUCTION: Currently, obesity is a major problem. Changes in dietary habits, regular exercise and bariatric surgery promote better quality of life. Massive weight loss may cause deformities in different body segments, which are normally corrected in multiple surgeries. We present a systematic way of treating these patients, which enable surgical procedures in different regions of the body in a single step. METHODS: This was a retrospective observational longitudinal study. The surgical technique consisted of gull-wing type incision on the back with resection of fatty tissue associated with abdominoplasty that promoted improvements in the pubis, back, buttocks and lateral thighs via a single incision. The combination of satellite excision as the thigh lifting was associated in certain situations as well as treatment of the upper segment of the trunk in a second time. RESULTS: The age of patients ranged from 24 to 52 years, average of 34 years old. The mean surgical time in the circumferential excisions was 4 hours and 8 minutes. Surgical complications observed were hematoma, flap necrosis and/or infection. There were three cases of small dehiscence and seroma and one case of pulmonary thromboembolism. In the outpatient follow-up, 85.7% of patients showed to be very satisfied with the final result and no complaints about the size and position of scars. Technical analysis showed a satisfaction level of 92.8% of cases. CONCLUSION: Satisfactory results elucidated the safety and feasibility of the technique, and scars were acceptable due to significant improvement in body contouring.

Anatomical study has proven vital to the understanding and improvement of rejuvenation techniques of the face and neck. The microscopic septa responsible for individual facial fat compartments are also present in the neck. The authors' anatomical studies of the neck, including supraplatysmal and subplatysmal elements, have influenced their surgical and nonsurgical techniques. Careful muscular resuspension and modification of both deep and superficial fat compartments can lead to impressive and lasting aesthetic outcomes. The authors present their algorithm and approach to both surgical and noninvasive methods for aesthetic neck contouring. The discussion contained here is augmented by video footage of injected, fresh cadaver dissection that highlights the anatomical relationships of neck fat compartments discussed in this article.

INTRODUCTION: There is a growing demand for weight loss sequelae. Some women have a particular morphology with a major trochanteric excess that warrants specific treatment. MATERIALS AND METHODS: On the basis of the technique published in 1964 by Pitanguy, the authors suggest a lift of the external face of the thigh with the scar redrawing the lower part of the buttock, from the gluteal fold to the inguinal region by crossing the trochanteric region. Eleven cases of major trochanteric excess after slimming are presented. In 4 cases, the trochanteric excess is solely treated by trochanteric under gluteus lift. In 7 cases, it is associated with abdominoplasty to perform a trochanteric under gluteus bodylifting. RESULTS: Complications and results are presented. The trochanterian excess is reduced, the buttock is harmonious and the patients satisfied. DISCUSSION: Modifications are made to the much criticized technique of Pitanguy. The lipectomy replaces the monobloc resection, the scar is verticalized on the trochanter to join the abdominoplasty scar. This technique is an alternative to the vertical trochanteric resection associated with classic bodylift. CONCLUSION: The trochanteric under gluteus lift is an elegant solution for treating large trochanteric excess. It is adaptable. It can become bodylifting TSF in the cases of great slimming.

Antecedentes y Objetivo. La lipoatrofia facial es una complicación frecuente de los pacientes con VIH/SIDA, y se asocia al tratamiento antirretroviral. Consiste en una pérdida anómala de la grasa facial, lo que supone un grave problema personal, laboral y social para estos pacientes, pudiendo provocar falta de adherencia al tratamiento. Para corregirla, cuando está establecida, es preciso el tratamiento mediante infiltración de materiales de relleno. El objetivo de nuestro estudio es evaluar la efectividad y seguridad del tratamiento de la lipoatrofia facial con relleno de gel de poliacrilamida. Material y Método. Realizamos un estudio de seguimiento prospectivo de 1 año en 15 pacientes con lipoatrofia facial grados II-III. Recogemos los efectos secundarios presentados. Analizamos la permanencia del relleno facial medido mediante tomografía axial computarizada y la evolución en el grado de lipoatrofia facial, en el grado de afectación del paciente y en la satisfacción de este tras la intervención. Resultados. En el 100% de los pacientes de la muestra no observamos efectos adversos al año de seguimiento. No encontramos un descenso significativo del espesor del relleno facial ni en la satisfacción de los pacientes al mes y al año del tratamiento. Existió una reducción estadísticamente significativa en el grado de afectación de los pacientes y en su grado de lipoatrofia facial tras el tratamiento, al mes y al año de seguimiento con p < 0.0001. Conclusiones. Según nuestros resultados, el gel de poliacrilamida es un material de relleno seguro y efectivo para el tratamiento de la lipoatrofia facial en pacientes VIH+ en situación clínica estable (AU)


Background and Objective. Facial lipoatrophy is a frequent complication of patients with HIV/AIDS associated with antiretroviral therapy. It consists of an abnormal facial fat loss, which poses a serious personal, occupational and social problem for these patients resulting in a lack of adherence to the treatment. Treatment by infiltration of filling materials is required to correct the established facial lipoatrophy. The objective of this study is to assess the effectiveness and safety of the facial lipoatrophy treatment filled with polyacrylamide gel. Methods. A one-year prospective follow-up study is carried out in 15 patients with facial lipoatrophy grades II-III. Side effects are measured. The permanence of the facial filler is analyzed by TAC and the evolution in facial lipoatrophy grades, in the degree of involvement of the patient and its satisfaction after surgery. Results. A 100% of the sample did not portray any side effects in the year of monitoring. A significant decrease in the thickness of the facial filler or in the satisfaction of the patients could not be found a month and a year after the treatment. There is a statistically significant decline in the degree of involvement of the patient and the extent of facial lipoatrophy after treatment with p < 0.0001. Conclusions. Polyacrylamide gel is a safe and effective filling material for the facial lipoatrophy treatment of in HIV+ patients in stable clinical situation (AU)

Lipoatrophy is a known, albeit rare, side effect of insulin injections. We describe a case concerning a 17-year-old woman with insulin-dependent diabetes, who was treated with an insulin pump. In the areas surrounding the injection point subcutaneous lipoatrophy occurred. Such lipoatrophy is of cosmetic importance, and it may incur glycaemic imbalance due to altered and unpredictable absorption of insulin in the atrophied areas. With no spontaneous regeneration of the atrophied areas observed the patient was successfully treated with lipofilling in the atrophied areas.

Upper arm contouring is based on the location and amount of excess skin and fat. The short-scar brachioplasty addresses minimal to moderate skin laxity and lipodystrophy in the proximal arm in patients with appropriate skin tone and quality. This article highlights technical refinements of the senior author's (R.J.R.) approach to short-scar medial liposuction-assisted brachioplasty to maximize results and minimize incision length. To highlight this simple and safe approach with high patient/surgeon satisfaction, the authors discuss the following in this Video Plus article: patient examination, preoperative assessment, surgical pearls, and postoperative outcomes.

This case report describes a female patient diagnosed with Barraquer-Simons syndrome, a rare form of acquired partial lipodystrophy characterised by symmetrical loss of adipose tissue from face, neck, upper extremities and the trunk with onset in early childhood. Initial symptoms were seen at the age of 8 years. Our patient did not show signs of renal impairment and this may be associated with the syndrome. Treatment of lipoatrophy in these patients is limited to cosmetic restoration, and autologous fat grafting has shown sustained positive effects with no or very little loss of volume at follow-ups. Furthermore, the treatment has resulted in considerable improvements in her quality of life and daily functioning. She has not experienced any adverse effects. Accurate and early diagnosis is important, and clinicians should consider early intervention for these patients. Autologous fat grafting is recommended as a safe procedure.

Brachioplasty has become one of the most rapidly growing operations in plastic surgery. There are a variety of arm presentations, due to either weight loss or other reasons, and the extent of the defect can carry into the lateral chest wall and the back. In this featured operative technique article, the author describes indications, patient selection, and criteria for classifying brachioplasty, and illustrates the surgical techniques of this procedure. Markings, scar positioning, and the various operative steps are described along with the intricacies of postoperative care, including scar control. A personal experience, along with the limitations of brachioplasty, are discussed.

A 70-year-old woman presented to our practice with profound ptosis of the left upper eyelid and notable asymmetry of the periocular area. On examination, she was noted to have significant atrophy of the periocular tissues on the left side, with lower eyelid retraction. These features were present but less severe on the right side. Upon further questioning, she stated that she had cataract surgery on the left side that was complicated by a high intraocular pressure and required subsequent secondary surgery. She had taken a prostaglandin eyedrop for many months after her cataract surgery to keep the eye pressure low. Recently, a newly recognized adverse effect of prostaglandin eyedrops has been described in the ophthalmic literature in which patients develop periorbital lipodystrophy. This case emphasizes that this may occur unilaterally in patients taking the eyedrop in only one eye, and should be recognized prior to considering functional and aesthetic surgery of the periocular area.

OBJECTIVE: The aim was to demonstrate a diagnostic challenge of sclerosing mesenteritis initially considered as liposarcoma. CLINICAL PRESENTATION AND INTERVENTION: A 45-year-old man was admitted with a painful abdominal mass. Abdominal computed tomography demonstrated a well- demarcated tumor in his left hemiabdomen, with a large fat component and areas of soft tissue attenuation suggestive of liposarcoma. Intraoperative findings showed a tumor arising from the greater omentum. The tumor was completely removed, and histopathology confirmed a pseudotumorous type of sclerosing mesenteritis with dominant mesenteric lipodystrophy. CONCLUSION: This case showed that a pseudotumorous type of sclerosing mesenteritis should be considered in the differential diagnosis of the mesenteric tumors.
